丁香五月婷婷中文 at Stark will host a Flu Shot Clinic on Thursday, Oct. 8 from 7:30 - 11:30 a.m. in the Recreation & Wellness Center.
This flu shot clinic is open to all students, faculty, staff and dependents.
If you are enrolled in one of the university-sponsored group health plans through Medical Mutual or Anthem, the cost of the flu shot is fully covered by the plan. You must bring your insurance ID card with you to the clinic. If you are not covered by one of the university's group health plans, you can still participate for $30 (check payable to 丁香五月婷婷中文).
Appointments and completed consent forms are required. Print and complete forms prior to attending the flu clinic for prompt service.
